The following forms are required by the FHSAA before a student-athlete may participate in any type of conditioning, workout, practices or contests and there may be additional forms required to participate. Please check with your districted high school athletic office for any additional formsAll forms must be turned in directly to the school athletic office.

These forms contain additional statements which are specific to the School District of Sarasota County and each individual school. These forms require the signatures of the parent/guardian and student-athlete.

Sarasota County Schools offers a variety of opportunities for students to participate in athletic endeavors, including competitive sports. The School Board of Sarasota County has adopted a resolution promoting good sportsmanship among student athletes and the adults who guide them. The resolution calls for the promotion of the highest standards of sportsmanship at all times and calls on coaches to convey the values of sportsmanship and good conduct, including fair play, respect, and graciousness in winning or losing. The document also cites the responsibility of high school principals to ensure that school athletes adhere to the high standards set forth by the Florida High School Athletic Association (FHSAA).
